Office Hours

We are open Monday through Friday 8:00am-5:00pm.

Prescriptions

All prescriptions and refill requests should be made during normal office hours. Please allow 24 hours for prescription refills and have your pharmacy call the office at (850) 897-3678 for renewal of medications.

Your Medical Records

Medical records are confidential. We will not release information about your care to anyone, including other family members, without a signed release.

Emergencies:

For life-threatening situations, call 9-1-1 or go to the nearest emergency room. If you have an urgent problem, please call our office for instructions. After hours, our answering service will inform you of how to reach the physician on call.
